What to Expect

Submit your request — Complete the form with your health concerns and preferred timing. There is no commitment at this stage.

Confirmation call — Our coordinator will call or email within one business day to finalize your appointment slot and discuss payment or insurance.

Pre-visit intake — You will receive our comprehensive health history questionnaire and consent forms via our secure patient portal (typically 20 to 30 minutes to complete).

Your visit — Arrive 10 minutes early. Bring any recent lab results, medication lists, and supplement inventories. Your practitioner will have reviewed your intake in advance.

Cancellation Policy

We require a minimum of 24 hours notice for cancellations or rescheduling. Late cancellations (under 24 hours) and no-shows are subject to a $75 fee. We understand emergencies happen and will always work with patients on a case-by-case basis. See our full practice policies for details.
